Online Rehabilitation after Knee Replacement
The weeks after a knee replacement decide much of how well the new knee works. Rehabilitation at home — with exercises supervised by video and progressed each week — lets you, or your parent, recover without travelling to a clinic while the knee is still sore.
Who it is for
- You have had a total or partial knee replacement and are back home
- Your surgeon has cleared you to start or continue physiotherapy
- You are struggling to bend or straighten the knee, or to walk without support
- A family member wants to help a parent with their exercises
What recovery involves
- Getting the knee to straighten fully and bend well
- Strengthening the thigh muscles, which weaken after surgery
- Walking with an aid, then without one
- Stairs, getting up from chairs, and confidence
How the online consultation works
What Dr. Bajpai checks on the video call
- How far the knee bends and straightens, on camera
- Your walking pattern and how you use the walker or stick
- Swelling — and reminders of when to call your surgeon about the wound
- Your home setup: chair height, bathroom and stairs
A typical treatment plan
- 1. Weeks 1–3
Early
Controlling swelling, straightening and bending exercises, and walking with an aid.
- 2. Weeks 3–8
Building
Strengthening, reducing walking aids, and stairs.
- 3. Weeks 8–12+
Returning
Longer walks, balance work and a return to normal activities.
How long it takes: Most people are walking independently within a few weeks. Strength and comfort keep improving for months — often up to a year.
What you can do at home
- Do your exercises little and often — several short sessions a day
- Use ice and elevation for swelling
- Rest with the knee straight — do not keep a pillow under the knee
- Follow your surgeon’s instructions on how much weight to put through the leg
When to get urgent or in-person care
- Calf pain, swelling or redness, or sudden breathlessness — possible blood clot, emergency care now
- Fever, redness or discharge from the wound, or rapidly increasing pain — call your surgeon today
- A fall onto the operated knee

Questions about knee replacement rehabilitation
When can I start physiotherapy after surgery?
Usually straight away in hospital, then at home after discharge. Always follow your surgeon’s protocol — Dr. Bajpai works to it.
Can an elderly parent manage online sessions?
Yes, with a family member to hold the phone and help with the setup. Many families find it easier than getting a parent to a clinic in the first weeks after surgery.
How long does rehabilitation take?
Intensive rehabilitation usually runs for the first 6–12 weeks, with exercises continuing at home after that. Progress keeps coming for months.
Is online rehabilitation as good as home visits?
For exercise-based rehabilitation, the trial evidence is encouraging: a 2023 meta-analysis of 37 trials found telerehabilitation for musculoskeletal problems comparable to face-to-face care. If you need hands-on help, Dr. Bajpai will tell you.
What the evidence says
- 37 clinical trials, 4,288 participants. Telerehabilitation improved every clinical outcome studied versus control (SMD 0.24-0.91), and when compared specifically against face-to-face rehabilitation the difference was not statistically significant — i.e. comparable, not superior. It was also USD 89.55 cheaper per person. Molina-Garcia et al., 2023
